Life, accident & critical illness claims

 

In order to make an insurance claim, you must be the insured, beneficiary or executor of the estate. If you’re not, we’ll guide you through the process.

Checklist

 

Information you’ll need to provide when you call us:

 

  • Policy number
  • Name of insured, beneficiary or beneficiaries
  • Cause of disability or death
  • If it’s a life insurance claim, place of death
We mail you the forms 
 

A customer service representative will assess your situation and send you the relevant claims package.

You complete the forms & return them 

Once you have completed the forms, return them to us by fax or by mail in the self-addressed, stamped envelope provided. Note: incorrect information or missing documents can delay a claim decision so please carefully review the materials to ensure that you have all the required documentation before sending them to us.

To protect your privacy and the security of your personal information, we can’t currently accept claims forms, notifications or correspondence by email.

We evaluate your claim & make a decision 

Our team of claims specialists will review your claim as soon as we receive it. We will ensure that the beneficiary, or the estate as applicable, will receive all the benefits it is entitled to receive under the policy. We’ll let you know if we require additional information or documents. After your claim is approved, we will mail a cheque to the beneficiary or estate, as applicable, within five business days.

To make a claim and for assistance before your departure or during a medical emergency while travelling, call Allianz Global Assistance :

Creditor insurance claims

 

To file a claim on a BMO Loan, Mortgage, Line of Credit or Commercial Lending Product, contact your local branch

 

To file a claim if you have BMO Credit Card Balance Insurance, call 1-800-268-5962.
