If you are unable to contact your advisor for some reason, please call us at 1-800-387-4483 or email BMO Insurance to discuss your policy.
You’ll need to provide a policy number so that we can verify that we can share the information with you.
Sign into your financial institution's online banking site or app where you normally pay bills.Add our company as a payee by entering the following information:
Payee Name: BMO LIFE ASSURANCE COMPANYAccount Number: 811 (plus your BMO policy number).
Note:The 811 is mandatory and needs to be included before your policy number.You'll need to enter a separate account number for each BMO policy that you are paying.If you need assistance in setting this up, please contact your own bank.
Complete and mail us the Change My Beneficiary Form on the Existing Customers page.
Submit a completed copy of the Name Change or Correction form.
Important: Remember to include your policy number, copies of required documents and signature on any correspondence you send to us.
