Credit Bureau Authorization, Terms and Conditions and Privacy Disclosures Applicable to Your BMO Credit Card Application
Credit Bureau Authorization
I consent to the Bank of Montreal (or the "Bank") obtaining credit information and other financially-related information about me from my employer, any credit bureau, any registry, any person who has or may have financial dealings with me, my telecommunications services provider or mobile services provider, and any references I have provided to the Bank to verify my identity. I consent to these third-parties disclosing this information to Bank of Montreal. The Bank may disclose such information to any credit bureau and to any person who has or may have financial dealings with me. The Bank will not obtain such information after my Cardholder Agreement ends, but may continue to disclose such information to maintain the integrity of the credit bureau system and credit granting process. I agree that if I am issued a Card, I am the only person permitted to use it. I have read the Terms and Conditions and the Privacy Disclosure and Consent below and agree to be bound by them.
Terms and Conditions
I, the Applicant, certify the information provided in this application is true and correct and request that you (Bank of Montreal) establish a BMO Mastercard Account in my name (the "Mastercard Account"). Please issue me the BMO Mastercard card (the "Card") applied for or such other card as you may approve, and send me renewals, substitutes or replacements issued from time to time at your discretion. I request a Personal Identification Number (PIN) to allow me to use the Card in automated teller machines accessible with the Card and to complete purchases at chip-enabled terminals. I request the services available to holders of the Card that may be issued to me and understand that such services may vary or be terminated and that some services or benefits may only be available to me if I enter into separate agreements or obtain separate authorizations.
I acknowledge that some services or benefits, including the BMO Roadside Assistance Plan, are supplied by firms independent of you and that you are not responsible or liable for anything in connection with those services or benefits.
If you issue me a Card, I agree to abide by the Cardholder Agreement, as amended or replaced by you, which you send relating to that Card, and I agree that use of any Card applied for will show I received the Cardholder Agreement.
I will be responsible for all use of, any fees for, or other charges to the BMO Mastercard Account.
If I am entitled to the BMO Roadside Assistance Plan, I consent to you providing Dominion Automobile Association any information reasonably required for the operation of the Plan.

Your Personal Information includes information you provided to us or information we collected from other sources about you, such as your name, address, age, financial data, Social Insurance Number, or employment records, and other information that could be used to identify you.

- verify your identity;
- ensure we have accurate information about you;
- understand your financial needs (including your eligibility for products and services you requested or accepted) and to manage our relationship;
- protect against fraud and manage other risks;
- communicate with you regarding products and services that may be of interest;
- understand our customers, including through analytics, and to develop and tailor our products and services;
- comply with legal or regulatory requirements, or as permitted by law; and
- respond to questions you may have.
If we use your Personal Information for a different purpose, we will identify that purpose.
Sharing your Personal Information
BMO Financial Group consists of Bank of Montreal and its affiliates. Your Personal Information, including information about your authorized representatives and beneficiaries, is shared within BMO Financial Group, to the extent permitted by law, to:
- ensure we have accurate information about you, and your authorized representatives and beneficiaries;
- manage our total relationship;
- provide a better customer experience;
- meet your needs as they change and grow; and
- manage our business.

Sharing: You may choose not to allow us to share account-specific information within BMO Financial Group, but you understand we will share your Personal Information where two or more BMO Financial Group affiliates provide you with jointly offered products or services.
